Clearly Define the Problem
Start by describing the issue in specific, measurable terms. Avoid vague statements and focus on what is happening, where it is occurring, and how often it is observed.
Contain the Immediate Impact
Before investigating further, take action to control the situation and prevent the problem from escalating.
Gather Data and Evidence
Collect relevant information to understand the problem fully. This may include process data, inspection records, maintenance logs, or direct observations.
Identify the Root Cause
Use structured techniques such as 5 Whys or Fishbone (Ishikawa) diagrams to explore potential causes. The objective is to move beyond symptoms and identify the underlying factor or combination of factors that led to the issue.
At this stage, it is important to validate the cause. A true root cause should explain the problem consistently and be supported by evidence.
Implement and Verify Corrective Actions
Once the root cause is confirmed, define and implement corrective actions that address it directly. These actions should be designed to prevent recurrence.
After implementation it’s important to monitor results to ensure the solution is effective and sustainable over time.

While RCA can be applied on its own, it is often most effective when used within a structured problem-solving framework such as 8D.
8D provides a clear sequence that guides teams from problem identification through to long-term prevention. Within this process, RCA plays a central role in identifying and validating the true cause of the issue, ensuring that corrective actions are based on evidence rather than assumption.
What makes this approach particularly effective is the connection between reactive and proactive improvement. Once a root cause has been confirmed and addressed, the learning does not stop there. It can be fed back into PFMEA to strengthen process controls, or into DFMEA to improve product or system design.
In this way, organisations create a closed-loop approach, problems are not only resolved, but used as a source of learning to reduce future risk.
